This is a legal document that is used while you are still living. Your HPOA names the person who can make healthcare decisions for you when you can't or no longer want to. Being married doesn't mean you are automatically your spouse's POA. Both of you will need these documents!

HOW IT WORKS:

1. Make your purchase using the drop-down menu below, checkout online, and pay for your service.

2. Schedule your 30-minute HPOA appointment. You pick your time and date and attorney Teshia Martin will call you on your scheduled time and date and ask you all of the questions she needs to create your HPOA. If you schedule with your spouse, make sure you are BOTH available for the appointment.

3. We will email you a copy of our Fee Agreement through Docusign and it MUST be signed.

4. We create your HPOA within a few weeks. We can either:

A - mail it to you with instructions on how to have it witnessed and notarized, or

B - bring it to you at an additional fee and go over it with you in detail and witness and notarize that day.
